Piano Intake Form

Non Piano Intake Form

Play It Forward accepts gently used instruments for children who desire to study an instrument but lack the resources to purchase an instrument.  Because our goal is to gift instruments that can be used by a student for up to 5 years, practicing 2 hours per day we do not accept pianos with the following flaws:

*chipped keys
*sticking keys
*broken strings
*sluggish action
*loose pins
*cracked soundboard
*missing music rack
*other

After donors have checked their pianos, they are required to fill out our intake form. This form gets sent directly to a piano technician, who will contact the donor for appraisal appointment. This appraisal will ensure the viability of the instrument and provide the donor with a valuation that can be used on their taxes. Donors are asked to pay the $50 fee directly to the technician, but PIF can pay his fee if donors are unable to afford the cost.

Once the piano is approved for Play It Forward, we pay to have the piano moved and tuned before it arrives at its next home.

If your piano does not meet our criteria, I will send you a resource list that may help you to find another option.

I look forward to taking the next steps towards getting your piano into the home of a student.
